Pay Per Call W-9

Dear Search and Rescue Member,

Below is a W-9 Form that is required by the IRS for us to have on file if you qualify for a 1099 or not. You are receiving this request for one of the following reasons:

• You are either a new vendor or payee
• We need updated information
• Have never received one previously

Please return the form by email to wcso_sar@washeriff.net , fax 435-656-6599 or you can mail / drop it off it to the Sheriff’s Office 750 S 5300 W, Hurricane, UT 84737. You will not be eligible for the Pay per Call program until this form is received. We will be unable to retroactively pay you for calls before this form is received.
